Massachusetts law requires that all incoming graduate and undergraduate students demonstrate that they meet the state immunization requirements. New undergraduates, including transfer students, can meet this requirement by completing the immunization portion of the student health certificate sent to all new undergraduate students. The Health Certificate can also be downloaded below. Graduate students are required to complete the graduate immunization form sent by their graduate school or downloaded below.
All undergraduate and graduate students are required by state law or the College to have the following immunizations:
To facilitate clinical placement all Health Science students are required to submit rubella serology and hepatitis B serology results in addition to the Hepatitis B and Rubella (MMR) immunizations. Varicella vaccine(s) or a varicella titer is also required.
Submitting your immunization record
If you have not already sent in your Health Certificate or immunization record you can download the necessary forms below. Please return all forms to the Health Center, 94 Pilgrim Road, Boston MA 02115 or by fax, 617-521-3467. Undergraduates should complete the Entrance Health Certificate, the Undergraduate Immunization Record and the Undergraduate and Graduate Meningitis waiver. Graduate students need complete only the Graduate Immunizations form and the Undergraduate and Graduate Meningitis waiver.
The Massachusetts Department of Public Health strongly suggests TB testing (PPD) for students who answer "yes" to at least one of the following questions:
